Let’s Start With the Basics: How Do Dermal Fillers Work?

Most dermal fillers are made with hyaluronic acid (HA), a sugar molecule the body produces to retain moisture, keeping the skin firm and supple. As HA levels decline with age, the skin loses some of its natural structure, making lines, folds and creases more apparent. 

Dermal fillers work by replacing what the body has lost to improve skin quality, enhance facial contours and restore youthful fullness. They introduce a cross-linked, stabilized form of HA into targeted areas, instantly replenishing volume for results that are visible right away. 

HA fillers are one of the most performed cosmetic treatments in the world, and the range of products and techniques has expanded considerably over the past decade. According to the International Society of Aesthetic Plastic Surgery (ISAPS) 2024 Global Survey, more than 6.3 million HA filler procedures were performed in 2024, making them second only to neurotoxins among non-surgical treatments. 

The increased demand has expanded the market considerably since the first HA filler was FDA-approved in 2003. Today’s patients have more products, more brands and more formulations than ever to choose from, each designed for different areas and treatment goals. 

While not strictly fillers, biostimulators like Sculptra® (poly-L-lactic-acid) and Radiesse® (calcium hydroxylapatite) have also broadened the conversation by offering another way to address age-related volume loss through increased collagen production. They tend to last longer than HA fillers (up to two years or more) and are a strong option for patients looking for gradual changes rather than an immediate result.

Facial Lines and Volume Loss Are Different Concerns

The expanding menu of options can make it easy to focus on the syringe instead of the strategy. 

Fine lines develop closer to the skin’s surface, while volume loss happens deeper, in the structural layers beneath it. Because these concerns originate at different depths, the products and techniques used to address them differ accordingly.

How Do Fillers Treat Static Lines?

Static lines remain visible even when the face is at rest. Unlike expression lines, which appear when the facial muscles contract, static lines become etched into the skin itself, shaped by collagen depletion, sun exposure, a loss of elasticity and years of repeated movement that turn temporary wrinkles into permanent creases.

Because static lines are shallower and more superficial than deeper facial folds, treating them calls for a lighter touch to ensure natural-looking results.

This is especially true with deeper folds around the midface and mouth, such as nasolabial folds and marionette lines. While most injectors will treat the fold directly, experienced ones also add volume in nearby areas to provide support and improve the way the tissue sits overall. 

For concerns like hollow temples, flattened cheeks or softening along the jawline, the strategy is usually less about softening a single crease and more about restoring contour, structure or balance.

Can Fillers Treat Expression Lines?

Sometimes, but they aren’t a first-line treatment. 

Expression lines appear when you smile, squint, frown or raise your eyebrows. Because these lines are caused by repeated facial movements, they are treated first with neurotoxins like BOTOX®, Dysport®, Xeomin® or Daxxify®, which temporarily relax the underlying muscles.

Fillers play a role when expression lines become visible at rest or when volume loss is making them more noticeable. In those cases, your injector may recommend a combination approach: a neurotoxin to soften the movement that creates the line and a small amount of filler to support the etched crease or restore balance in the surrounding area.
