What Is Facial Fat Grafting?

Facial fat grafting, also called fat transfer, restores lost volume by relocating your own fat from a donor site on the body, typically the abdomen or thighs, and injecting it into areas of the face that have become hollow or deflated over time. Because the material used is your own tissue, there is no risk of rejection or allergic reactions, and the results are long-lasting.

Facial fat grafting is primarily used to address:

  • Sunken cheeks and a loss of fullness in the mid-face
  • Temple hollowing
  • Weight loss-related facial changes, such as a gaunt appearance
  • Shallow or sunken areas along the jawline

Fat grafting addresses volume loss. It does not lift or tighten skin.

What Is a Traditional Facelift?

A facelift, also known as rhytidectomy, is a surgical procedure that repositions and tightens the underlying tissues while removing excess skin to restore firmer, more youthful contours. It is designed to improve moderate to severe signs of aging in the lower face and neck, including sagging, jowls and neck laxity. It is one of the most effective options for rejuvenating your appearance without losing the character of your features and expressions. 

A traditional facelift can address: 

  • Sagging skin along the cheeks and mid-face
  • Jowling and loss of jawline definition
  • Deep creases between the nose and mouth
  • Loose or banded skin along the neck
  • Facial tissues that have shifted downward over time

A facelift addresses tissue descent and skin laxity. It does not restore volume.

Who Is a Candidate for Each Procedure?

Good candidates for either facial fat grafting or facelift surgery are generally healthy nonsmokers with realistic expectations about what surgery can achieve. Skin quality, facial anatomy, recovery tolerance and the specific concerns you want to address all play an important role in determining which approach makes the most sense.

You may be a good candidate for a facelift and/or facial fat grafting if you:

  • Are in good overall health
  • Do not smoke, or are willing to stop before and after surgery as directed
  • Have realistic expectations about what surgery can and cannot achieve
  • Want a longer-lasting solution than non-surgical treatments can provide
  • Have enough recovery time to heal properly
  • Want a treatment plan tailored to your anatomy, concerns and goals

Most patients considering a facelift are in their late 40s, 50s or 60s, when skin laxity becomes more noticeable, while facial fat grafting is often performed in slightly younger patients who are beginning to notice a loss of fullness but may not yet need surgical lifting. That said, age alone does not determine candidacy. Some patients are best served by one procedure, while others benefit most from a combined approach.

How Do I Decide Which Is Right for Me?

When trying to determine whether a facelift or facial fat grafting is right for you, the real question isn’t, “Which is better?” It’s what changes you’re seeing. Are you noticing volume loss, skin laxity, or a combination of both?

When Volume Loss Is the Main Concern

If your face looks thinner, flatter or more hollow than it used to, volume loss may be the main driver. This is especially common in the cheeks, temples and under-eye area. Weight loss, especially rapid weight loss from GLP-1s, can make those changes even more noticeable, accelerating the signs of facial aging. The American Academy of Facial Plastic and Reconstructive Surgeons (AAFPRS) reported a second consecutive year of 50% growth in fat grafting procedures in its 2025 survey, driven by patients experiencing “Ozempic face.” 

When Skin Laxity Is the Main Concern

If your concern is jowling, lower-face heaviness, or a loss of jawline definition, a facelift is usually the stronger option. That is because the issue is not simply that the face has lost volume. It’s that the underlying tissues have descended, shifting the balance of your facial contours. When skin laxity is the primary concern, the goal is to tighten the underlying muscles, remove sagging skin and restore support in a way that looks natural and harmonious.  

Can They Be Combined?

Yes, and at The Swan Center, combining procedures when clinically appropriate is a common approach. A facelift firms and tightens the structural foundation of the face, but it doesn’t replace lost volume. Fat grafting performed alongside a facelift allows the surgeon to restore fullness where it’s been lost while addressing skin laxity, producing a more complete result than either procedure achieves on its own.

This is one of the reasons patients consistently describe our results as balanced and natural-looking. Addressing laxity without accounting for volume loss can leave a patient looking tight but hollow, while adding volume without lifting can look incongruous on a face with significant sagging. The most refined outcomes account for both.

Which Lasts Longer: A Facelift or Facial Fat Grafting?

Both facial fat grafting and facelift surgery offer long-lasting results. While neither will stop the aging process, they provide permanent volume and structural changes that can help you maintain a more vibrant appearance for years to come.

After facial fat grafting, about 30% of the injected fat is naturally absorbed, while the other 70% integrates with the body and behaves no differently than other fat. It is still subject to weight fluctuations but provides stable, long-term volume when paired with a healthy lifestyle.  

A facelift provides a permanent “reset” of sagging skin and tissues. Your face will continue to age naturally from that new baseline, but you will still look younger than if you had never had the procedure. Facelifts can last 7-10 years or longer, depending on factors such as smoking, sun exposure, skin quality and the surgical technique used.
